Membership  Applications  &  Bayou  Certified  Certification  Forms 

Click  on  the  application/form  link  of  your  choice,  located  at  the  bottom  of  the  page.  
Fill  out  the  form  completely  with  all  requested  information.  Incomplete  Bayou  Certification  forms
will  be  returned  unprocessed  unless  accompanied  by  an  explanation  as  to  why  information  was  left  blank. 
After  you  have  filled  the  form  in,  highlight  that  form  &  click  print.
Send  printed  form  along  with  appropriate  fees  to  address  listed  in  table  below.

INDIVIDUAL  YOUTH  MEMBERSHIP:  Covers  1-2  youth  in  the  same  family  -  Please  fill  out  a  separate  application  for  each  youth.

$20.00  per  youth  per  year
FAMILY  YOUTH  MEMBERSHIP:   Covers  3-5  youth  in  the  same  family  -  Please  fill  out  a  separate  application  for  each  youth.  Covers  youth  only.

$50.00  per  3-5  youth  in  a  family  per  year 
YOUTH  BREEDER  FEE:  Youth  must  be  a  current  paid  JLGA  Member  in  good  standings.

Each  youth  in  a  family  membership  that  wishes  to  be  listed  as  a  breeder  must  purchase  separate  breeder  memberships.

$5.00  additional  per  youth  per  year  
ADULT  BREEDER  FEE:  Covers  2  adult  breeder  members  in  1  family  or  a  partnership  of  2  adults  -  Additional  Adult  Breeder  membership  must  be  purchased  separately.

$30.00  per  year
BAYOU  CERTIFICATION  FEE:  Incomplete  forms  will  be  returned  unprocessed  unless  accompanied  by  explanation  of  incomplete  information.

$10.00  per  goat  -  good  for  the  life  of  the  goat.
